Being prepared for your visit will help speed up your registration and the amount of time spent in the clinic. We appreciate the urgency of your medical visit and do what we can to  get you seen by the doctor quickly and efficiently. Our typical visit times are under one hour from the time you arrive to the time you are discharged. Photo ID
The patient must present a legal form of identification upon check-in. Valid identification includes a driver’s license, passport, military ID, employment badge or school ID card.

Insurance Card
If patient is paying through insurance, they must bring a current insurance card.

Payment Policy

It is the policy of Trinity Urgent Care to have a Financial Policy that clearly outlines patient and practice financial responsibilities. We are committed to providing our patients with the best possible medical care and minimizing administrative costs. This Financial Policy has been established with these objectives in mind, and to avoid misunderstanding or disagreement concerning payment for professional services.

Insured Patient - Copayments, coinsurance and any unmet deductibles are due at time of service.

Cash Payers/Uninsured - Office Visits start at $120 and is due at time of service. If other services are performed in addition to the office visit (such as x-rays, procedures or labs) a discount of 30% will apply to additional fees.

Financial Policy - Payment plans are available with a credit card and bank account on file. A minimum or $120 is due on the date of service. Patients are expected to pay for professional services at the time of service unless prior arrangements have been made with us.

Forms of Payment - Payment for professional services can be made with cash, debit, MasterCard, Visa, American Express or Discover. Due to the high cost of returned checks we do not accept checks at this time. Payment plans are available with  credit card and bank account on file.
